Course Overview
In this CHCSS00132 – Individual Support – Home and Community (Disability) skill set, you will gain a deep understanding of the specific needs and challenges faced by individuals with disabilities in the home and community environments. Upon successful completion of this CHCSS00132 Individual Support – Home and Community (Disability) Skill Set, you will be equipped to work as a skilled support worker in various settings, including community care, group homes, and residential facilities. Your expertise will be highly sought after by disability support agencies, healthcare organizations, and community service providers.
Target Group
This skill set is for individuals who hold a qualification at Certificate III level or higher in Community Services, Health or a related field, work in various care and support roles and who wish to develop skills in home and community support for older people, within the delegations of an established individualised plan.
Students must satisfy the following age, English, and Academic requirements:
- Age: must be 17 years or over
- Undertake Pre-Training Review
- Achieve satisfactory result of LLN assessment: Learning, reading, writing, oral communication, and numeracy (ACSF Level 3) are desirable to participate in the course.
- Moderate level of digital literacy and has access to a computer with internet access.

Delivery methods:
The range of delivery methods may include, but is not limited to the following:
- Lectures / Instructions
- Pre-reading
- Demonstrations and modeling
- Practice opportunities
- Brainstorming activities
- Group discussions.
- Guided facilitation of individual or group learning activities, group work or project-based case studies.
Delivery Structure:
- The delivery structure is comprised of classroom training sessions, structured learning such as training support, learning activities, self-paced (to allow the learners to absorb and reflect on their learning), workshops, and work placement.
- The unit of competency will be delivered and assessed as a stand-alone unit.

These units provide credit towards a range of qualifications in the CHC Community Services Training Package at Certificate III or Certificate IV level.
Assessment methods may include:
- Knowledge Questions
- Written Assignment & Case Studies
- Projects / Research Reviews
All assessments will be conducted in accordance with the Training Package requirements, Principles of Assessment, and Rules of Evidence (https://www.asqa.gov.au/standards/chapter-4/clauses-1.8-1.12)
Assessment Conditions
For CHCCCS031 – Provide individualised support unit tasks outlined in the performance evidence must be demonstrated in simulation prior to being assessed in the workplace.
To be eligible for the award of a Statement of Attainment for the CHCSS00132 Individual Support – Home and Community (Disability), participants must successfully complete four units comprising:
- Five (5) Core units
Where the full course is not completed, a Statement of Attainment will be issued for any completed unit/s.
- CHCCCS031: Provide individualised support
- CHCCCS036: Support relationships with career and family
- CHCCCS037: Visit client residence
- CHCCCS038: Facilitate the empowerment of people receiving support
- CHCDIS020: Work effectively in disability support
